| It came from a person noticing another person was mad and then going to confirm it by asking if the person who appears mad was in fact angry. The person who asked the question didn't feel like talking in complete sentences with perfect grammar, so instead of saying "Are you mad?" he instead said simply "You mad?" |

I have full right to use a meme when I don't know where it came from as long as I know what it means.
In this case it literally means "Are you mad?", and even if that's wrong, it's so misused now it doesn't matter. |
