Act II is kind of spooky. The end of Act I had that sort of vibe, too. If you're interested I could spoil a tiny bit.

The Core:

After Slaya drops a scroll, it details that the Core can be opened, and that Green Man's Empress lies beyond there. Upon entering the core, a sci-fi-esque segment of an otherwise high fantasy inspired game, you fight your way to the underground segment.

Creations of the Core:

Stories from ages ago talked about a being that ruled the Core before Heartstrings had invaded. He is now a mess that deforms into a pool of blood and guts, but still has answers to where Heartstrings lies.

Gatekeeper:

Upon reaching the location told by the Lord, a large, ax-toting soldier stands within your way. Bearing the Green Man armor setup and such, he poses as a major threat. Upon defeat, you use his ax to open the door into the last part of your journey.

Dethroned:

You reach the center of the core, and it's revealed that Heartstrings has reduced to a literal heart with tubes that resemble strings. She constantly uses her forces to try and stop you, and when you kill her, she just implodes.

Act 0:

Invasion:

After confirmed reports of Green Man Forces heading into OT were heard all around OT, the OTers were forced to run to shelters. Others decided to stay back and find some items to make an alliance together to counter Green Man Forces. After a successful defense of the alliance's newly built base, you hear of a machine called Dreadnaught G4 riding into major OT Cities and grinding them to dust.

Assault on the Dreadnaught:

Your comrades setup a Mech to go patrol the city, and said Mech is ambushed by a group of Green Man Forces. You and your allies decide to take some heavy weapons and fight back the Dreadnaught. Heartstrings is angered by this, and decides it's time to have the one responsible go to OT Hell.

Descent:

Heartstrings ambushes the base and murders every comrade within it. You are eventually held as a sacrifice, and descend into OT Hell.

Act I:

Into Town:

After awakening, you get new weapons and armor, and make your way into town. Things are way different down here than they are above.

Local Troubles:

A Merchant is in grave need of someone to go into an old Scrapyard and rat out a man that calls himself "Scraplord". Needless to say, you wander into this horrid dungeon in the next quest. But before, a Disciple of Heartstrings invades.

Scrapyard Entrance:

After walking into this horrifying yard filled with machines that want to tear you open, you finally find a hatch that lowers into a basement lair.

Scraplord:

Upon entering the basement, you find the Scraplord resting upon his throne. He is enraged the Merchant sent you down there, and fights you in a Mech. After it implodes, he drops a fragment, and this was what the Merchant was looking for. You take a portal to Town.

The Messenger:

Upon entering, the Merchant is thankful that you were able to recover this fragment. He gives you a new piece of armor, and sends you on your way. Until suddenly, while you're in The Haven of Old OT, a cloaked messenger arrives. He raises skeletons, and forces that they attack you. After dealing with the skeletons, you question his hostility. He states that, "The Heart and her Servant sent me." to which a large skeleton rises from below. After defeating the Skeleton, you recover a note.

Messenger's Spell:

You come to find out that the note was a spell to awaken a large beast. As you go to vanquish the note, a Local Necromancer stops you, and reads the summon. You fight a massive beast, and then impale it completely. The Necromancer turns around, and unveils his hood. It was the Servant himself, Kistvaen.

Kistvaen:

After confronting the Servant, he raises an army of Undead that attacks Locals. It is only you and him in a ring of embers. Once you finish him, he drops a portal scroll and note, which can take someone into an Ancient OT Cave.

Act II:

The Caves:

After escaping town before you get turned into a pool of giblets, you come to find that this massive cave is home to a group of Dragons. Exploring each route of the cave leads into a room where a piece of lore is uncovered. The lore states that the Dragons that inhabit this cave wield a tooth that one can use to find an old kingdom. Said kingdom is home to a long-time warrior.

Tooth of the Dragon:

After exploring the cave, you see a large stone wall with signs on it. The signs depict that "Only the hopeless can finish this". You choose to enter, and the Wandering Dragon asks who disturbs his eternal rest. When he finds out a Fallen has, he decides to go on a murderous rampage. After impaling a spear into his head, he drops a tooth, and this tooth can be put into a symbol within the wall behind the corpse.

Kingdom in Corruption:

You descend into a dead Kingdom, where a Possessed Empress is the overseer. After wandering the city, you go into an old home. This old home has a symbol that would normally tell one to go to the Graveyard to redeem himself to the Slayer, but points you towards the highly defended castle. You find a rebellion that is able to fight their way into the castle so you can kill the Empress. The Empress drops a crest that opens the Graveyard.

Graveyard Run:

Upon entering the graveyard, it appears that a Cultist that follows Heartstrings has been messing around with skeletons. He eventually sends a massive one to fight you for entering his lair. Upon killing the skeleton, he tries to kill you himself. After impaling him, he drops a cross that takes you to the Slayer's Church.

The Slayer's Church:

After exiting the Graveyard, you go down into an Old Church, and find that a Priestess has been trying to awaken the Slayer's blade. The Priestess appears to be very unhappy that you came here during the Ceremony, and uses Holy powers to stop you. After a long battle, she gives up, but the Blade had already awakened, thus the Church falls apart.

Blade of the Slayer:

The blade, consisting of fragments from that medallion in Act I built into its base. The blade decides that you would be a worthy candidate to be in its possession. You eventually fight the blade to the point where its ghost comes out of it, and you then find out from the ghost itself, that Slayer's lair is down within the remains of the Church.

The Unslayable:

After going downwards into a lair, the Slayer himself asks the reason of your presence. After you state that you are here to end him, he becomes enraged, and starts to fight. After defeating Slayer, he drops a note that leads to the Heart of Strings, who rests within the core.

I'm considering having the Merchant dude as a Companion. Probably can do a side-quest for disassembling the blade where the medal can be complete.

Bonus: If you do the Medallion questline with the Merchant, then you can use the Medallion to easily defeat Heartstrings. But there's a second phase called Kernel Panic mode, where the boss becomes very powerful.
