"we all fall down" refers to death and i'm not entirely sure but "ashes, ashes" might be referencing cremation

| Posy refers to the stuffing in the masks doctors wore. It was made of sweet scents like flowers and honey because they thought smelling the rotting tissue of the infected gave you the plague |

The 'ring around the rosies' part represents one of the plague's effects, which was a red rash that appeared on the victim's skin.
Posies and other sweet smelling flowers were believed to have warded off illnesses, and were typically stuffed into the bird-like Plague doctor's mask.
'Ashes, ashes' refers to cremation, and 'We all fall down!' is everyone's eventual death.
It's still pretty catchy.
