they take the texture from the site
then they use an image editor like photoshop or paint.net or something and edit it then upload it |

ok so you get a texture (its like a decal) from the site
then you draw over or edit the texture
then you find a blank mesh
and you like upload your texture
and stick in to the mesh |

you go to like a free models and find a model of the original hat
and click properties
there you change the original texture ID (link) to the texture you made |
